What is Acute Paraproctitis

Acute paraproctitis is an acute purulent inflammation of the perirectal tissue, which occurs due to the penetration of infection from the anal glands or microcracks in the rectal mucosa into the surrounding tissues.

It is one of the most dangerous proctological diseases: it develops rapidly, accompanied by severe pain, fever, and a general deterioration of the condition. Without urgent medical assistance, the abscess may rupture into the abdominal cavity or cause a systemic infection (sepsis).

👉 The only effective treatment method is urgent surgical incision and drainage of the abscess.

Symptoms of Acute Paraproctitis

Acute throbbing pain in the anus and rectal area

Redness, swelling, and hardening of tissues around the anus

Increased body temperature up to 38–39 °C

Chills, weakness, signs of intoxication

Pain when sitting, walking, and during defecation

Sometimes — purulent discharge from the rectum

⚠️ If these symptoms appear, it is necessary to urgently consult a doctor. Self-treatment or attempting to open the abscess is unacceptable and life-threatening.

How the Surgery for Acute Paraproctitis is Performed

Surgical treatment is performed urgently — within the first 24 hours after diagnosis.

The procedure is performed under local, spinal, or general anesthesia and takes 20–40 minutes.

Stages of the operation:

Diagnosis and determination of the abscess location — digital examination, ultrasound, or sigmoidoscopy.

Opening of the purulent focus — an incision is made in the skin or mucosa over the abscess.

Drainage and removal of pus — the cavity is thoroughly washed with antiseptics, and drainage is installed if necessary.

Search and elimination of the primary infection focus (anal crypt) — reduces the risk of recurrence and fistula formation.

Application of a sterile dressing and prescription of postoperative therapy.

The patient usually remains in the hospital for 1–2 days for observation and dressing changes.

Postoperative Period and Recommendations

Regular dressing changes and wound treatment with antiseptics

Prescription of antibiotics and pain relievers

A diet high in fiber for soft stools

Sitz baths with antiseptics

Limitation of physical activity for 7–10 days

Follow-up with a proctologist and ultrasound monitoring of healing

Full recovery usually occurs within 2–3 weeks.

In some cases, after acute paraproctitis, a chronic rectal fistula may form, requiring planned surgical treatment.

Why It Is Important Not to Delay the Surgery

✅ Eliminates the source of infection and prevents its spread

✅ Reduces the risk of chronic paraproctitis and fistula formation

✅ Quickly relieves pain and symptoms of intoxication

✅ Helps avoid severe complications — peritonitis, phlegmon, sepsis

💡 Surgical treatment of acute paraproctitis is an urgent, life-saving procedure that eliminates purulent inflammation, prevents dangerous complications, and restores comfort and health to the patient. Timely intervention is the key to quick recovery and minimal risk of recurrence.
