CHECKUP PARKINSON'S DISEASE

A comprehensive check-up helps identify possible causes and risk factors, assess the condition of the nervous system, exclude diseases with similar symptoms, and detect signs of neurodegeneration at early stages.

CHECKUP PARKINSON'S DISEASE | Comprehensive examination
Parkinson's disease develops gradually, and the first changes may appear long before pronounced motor symptoms. A check-up allows timely initiation of treatment, slowing disease progression and maintaining a high quality of life. Our check-up includes:

Laboratory and Instrumental Tests

- Ceruloplasmin
- Copper
- Thyroid-stimulating hormone (TSH)
- Free thyroxine (Free T4)
- Vitamin B12 (cyanocobalamin)
- Vitamin D (25-hydroxyvitamin D, 25(OH)D)
- Folic acid
- Homocysteine
- Ionized calcium
- Magnesium
- Blood biochemical analysis:
- ALT
- AST
- Creatinine
- Urea
- Ferritin
- Serum neurofilament light chains (Neurofilament Light Chain, NfL) (Rödger laboratory, Germany)
- Serum amyloid A (SAA)

Cardiovascular Ultrasound Diagnostics:

- Echocardiography with Doppler
- Ultrasound Doppler of the head and neck vessels

Instrumental Diagnostics:

- Brain MRI (1.5–3 Tesla) with intravenous contrast

Neurologist Consultation:
- Assessment of early non-motor symptoms
- Sleep disorder screening (presence of shouting, talking, arm and leg movements during sleep)
- Evaluation of autonomic nervous system function (orthostatic test: blood pressure measurement in sitting and standing positions)
- Gastroenterological status assessment (presence of chronic constipation of unclear origin)

Since this check-up includes assessment of protein metabolism, trace elements (ceruloplasmin, copper, iron), B vitamins, and hormones, the following recommendations must be followed to obtain reliable results:

Diet and Drinking Regimen

- Blood must be drawn strictly fasting in the morning after 8–12 hours of fasting. For example, if dinner was at 8:00 PM, the optimal time for blood sampling is between 8:00 AM and 10:00 AM.
- 24 hours before the test, avoid fatty, fried foods, and fast food. Excess fat in the blood may cause serum turbidity (chylemia), which complicates or makes it impossible to perform some laboratory tests.
- On the morning of the test, it is recommended to drink 1–2 glasses of clean, non-carbonated water. This facilitates blood sampling, lowers blood viscosity, and reduces the risk of hemolysis.
- Tea, coffee, juices, and other beverages are not permitted.
- Medications taken may significantly affect test results.

B Vitamins and Biotin:
- It is recommended to stop taking supplements containing vitamin B12, folic acid, and biotin (vitamin B7) 3–5 days before the examination. High doses of biotin can interfere with immunological tests, including TSH and free T4 levels.

Iron and Copper Supplements:
- If you are taking iron or copper supplements, it is advisable to discontinue them 5–7 days before the examination (unless otherwise directed by your physician).

Ongoing Therapy:
- Medications for blood pressure control, heart rate, as well as levothyroxine, should ideally be taken after blood sampling on the day of the examination, unless otherwise instructed by your doctor. Do not stop life-saving medications on your own.

Lifestyle Before the Examination

- Completely avoid alcohol intake 24 hours before the examination.
- Do not smoke (including e-cigarettes and heated tobacco products) for at least 2 hours before blood sampling.
- Avoid intense physical activity, strength training, long-distance running, and heavy lifting for 24 hours before the test, as these may affect enzyme, electrolyte, and other laboratory parameters.
- Arrive at the clinic 15 minutes before your appointment and sit calmly before the procedure. This will help normalize blood pressure and pulse and reduce stress impact on test results.
